Faculty Supervision Request Form

When contacting faculty to request a supervisor confirmation letter, please only contact faculty who have research fields you feel relate to your own research. Be sure to only contact one faculty member at a time and address the faculty directly when emailing them. Please use the text box provided below to write an introduction to your desired supervisor.

It is recommended to contact a faculty supervisor well in advance, at least one month before the application deadline, to ensure enough time for the application process.

INSTRUCTIONS:

  1. ● Please only specify faculty who have research fields you feel relate to your own research. Be sure to only submit one faculty supervision request at a time.
  2. ● Ensure to include a self-introduction for your desired supervisor.
  3. ● It is recommended to submit your faculty supervision request well in advance, at least one month before the application deadline, to ensure enough time for the application process.
